Welcome back! We often get asked: "If exosomes come from stem cells, why not just use those instead?" Great question, here's the short answer.
Living Cell vs. Packaged Cargo
A mesenchymal stem cell (MSC) is alive. It can sense what's happening in the body, like low oxygen, inflammation, or injury, and will respond accordingly. An exosome is different: it's a small particlereleased by a cell, not a living cell itself. Once it's made, it can't sense oradjust to your body afterward. It just delivers whatever it was packaged with.
Exosomes Aren't FDA-Approved
This matters more than it might sound. As of now, there are no FDA-approved exosome products, and the FDA treats themas regulated drugs requiring approval before clinical use. Same as Stem Cells!
In 2019, several patients became seriously ill, including sepsis, after receiving unapproved exosome treatments, prompting an FDA public safety warning. The agency has continued issuing warnings to exosome manufacturers and clinics since then. Clinics claim, “it isnot cells so it is legal!”
MSCs Have Decades of Safety Data Behind Them
MSCs, by contrast, have a long track record. They were first described in the scientific literature back in 1974 and have since been studied in many clinical trials.
One well-known review pooled data from 8 randomized trials and 321 patients and found no link between MSC treatment and serious complications like infection, organ problems, or malignancy. The only notable finding was mild, short-lived fever. The Bottom Line
